Confabulation

Definition

Unintentional production of false or distorted memories without conscious intent to deceive. The person may be confident in the account.

  • Merck: Korsakoff psychosis consists of mental confusion, dysphonia and confabulation with impaired memory of recent events; it probably results from chronic thiamine deficiency and may follow repeated episodes of Wernicke encephalopathy (1).
Assessment

Obtain collateral history; assess attention, orientation, memory encoding/retrieval and executive function. Review alcohol use, nutrition, neurological disease, head injury and medications. Examine for delirium and focal neurology.

  • See Delirium, Dementia and Mental State Examination pages.
Red flags

Acute onset, fluctuating attention, reduced consciousness, focal deficits, seizures or suspected thiamine deficiency requires urgent medical assessment.

  • See Wernicke–Korsakoff page for thiamine treatment.
Associations

Korsakoff syndrome, frontal-lobe injury, dementia and other amnestic syndromes. Differentiate from delusion, lying and ordinary memory error.

Management

Treat the underlying cause, correct nutritional deficiency when suspected according to emergency protocols, use memory/environmental supports and communicate non-confrontationally.
